The following forms are for standard reporting only and can be submitted electronically. If you are required to report loan-level detail for New Insurance Written and portfolio performance, please submit reports in the format as described in your term sheet.

Reports for New Loans and Lines

• Report of New Loans Form (single premium) - new home equity loans to be insured for a specified time period under a closed-end policy
• Report of New Loans Form (monthly premium) - new home equity loans to be insured for the month under a closed-end policy
• Report of New Lines Form (single premium) - new home equity lines of credit to be insured for a specified time period under a revolving (open-end) policy
• Report of New Lines Form (monthly premium) - new home equity lines of credit to be insured for the month under a revolving (open-end) policy

Condition of Accounts Reports

• Monthly Condition of Accounts (single premium) - condition of outstanding home equity loans, including delinquency and additional detail on 90+ days delinquent loans (please complete separate forms for loans and lines of credit)
• Monthly Condition of Accounts (monthly premium) - condition of outstanding home equity lines of credit, including delinquency and additional detail on 90+ days delinquent lines (please complete separate forms for loans and lines of credit)
